Omnipod 5 (starter kit)

CONDITIONS:

• Boxes must be in mint condition — no rips, holes, tears, or marks from pens, markers, or other writing instruments.

• Do Not Remove RX stickers— Please allow us to do so.

• All original sensors must be included — the date and lot number on the sensors must match the date and lot number printed on the outside of the box.

Expiration Date Requirement:
To determine if your Omnipods meet our expiration date requirement, follow this method:

1. Count the current month as “0.”

2. Each following month increases by one (e.g., if the current month is June, July is “1,” August is “2,” and September is “3”).

3. Your Omnipods must have an expiration date that is at least three (4) months from the current month.
